We currently have positions available for a Discharger Planner at Del Amo Hospital. Del Amo Hospital is a 166-bed behavioral health facility that is located in the South Bay area of Los Angeles.

The Discharge Planner is responsible for making collateral contacts and arranging discharge plans/linkage with aftercare services. Additional responsibilities include the provision of clinical services to psychiatric inpatient and outpatient patients and their families, completion of safety interventions and individualized aftercare plans including family sessions. The Discharge Planner serves as a member of the interdisciplinary team and attends appropriate clinical treatment team meetings on behalf of patients and their families.
Job responsibilities include but are not limited to:
Coordinates individualized discharge plan for the patient with assistance from the treatment team.
Coordinates with Utilization Review for aftercare appointment information and is familiar with Utilization Review process.
Coordinates with Social Work Staff in compiling accurate referral source, contact database and previous treatment providers.
Attends Treatment Team meetings on units as directed by Supervisor.
Works collaboratively with the physicians, nurses and social workers and other team members to influence timely and smooth discharge with appropriate care and positive outcomes; presents alternative option and services as needed.
Maintain statistical report of services provided; submits to supervisor as requested and/or the Director of Quality Assurance
Ensures that the discharge documentation (i.e. Discharge Checklist, Safety Plan, Aftercare form) is complete prior to patient discharge.
Consistently refers patients to appropriate community agencies.
Demonstrates ability to conduct family and community interventions appropriately.
As requested, assists patients and families in understanding, accepting and following medical recommendations
