Conducts patient/family education that is specific to psychosocial needs and continuity of care and includes the family/significant others when appropriate.
Evaluates and documents the patient's response to the Plan of Care and to interventions.
Revises and updates the Plan of Care as evidenced by documentation.
Documents patient's response to continuum of care alternatives.
Provides supportive counseling and grief and loss crisis intervention.
