Providence Medical Group is looking for a Behavioral Health Specialist to join the Collaborative Engagement Team. This position will be for an exempt full time position, Monday - Friday during the day.
In this position, you will:
Independently practice in an evaluation or treatment setting dedicated to evaluation and treatment of psychiatric and substance use disorders.
Collaborate with patient/support system in evaluating, diagnosing patients utilizing current edition Diagnostic and Statistical Manual of Mental Disorders and/or providing specialized psychotherapy services as described in CPT to reduce and/or resolve symptoms of psychiatric and/or substance use disorders identified.
Complete assessment of patient risk to self and/or others, complete and implement risk reduction/mitigation plans to ensure patient and community safety, utlizing established approaches and standards.
Incorporate evidence based approaches in psychotherapy including but not limited to Cognitive Behavioral Therapy, Motivational Interviewing, Dialectical Behavioral Therapy, Acceptance and Commitment Therapy, and the Recovery Model.
Demonstrate effectiveness in assessment and diagnosis, including crisis evaluations, individual, group and family psychotherapy and multi-modal treatment settings.
Demonstrate knowledge, skills and abilities of advanced social work practice.
Perform duties in a manner that respects patient self-determination, reduces health disparities/barriers to care, and adheres to the NASW code of ethics, Standards of Practice (SOP), and the Providence mission.
Responsible for identifying and engaging patients within the primary care population with high risk behavioral health and substance abuse conditions.
Collaborate effectively with primary care treatment team to develop effect care plans that outline treat to target metrics identified from valid measurement tools to assess and document baseline symptom severity.
Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatments as appropriate.
Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider.
Required qualifications for this position include:
Master's degree in Social Work from a CSWE Accredited School of Social Work or from a school in candidacy status by the Council of Social Work Education (CSWE)
Lincensed Clinical Social Worker (LCSW) with the State of Oregon
Valid driver's license and reliable transportation to homes of patients as needed
American Heart Association Basic Life Support(AHA-BLS) for Healthcare Provider completion within 1 month of hire
Certified Alcohol and Drug Counselor (CADC) for those in a dedicated Chemical Dependency treatment program, or 60 hours of approved continuing education units (CEUs) in diagnosis and treatment of substance use disorders within one year of hire
Certification to assess and implement a Directors Hold as delegate to Mental Health Authority within 6 months of hire
2 years of post Master's experience in a behavioral health treatment setting
The Project Nurture program has been created to address the need for high-level, team-based integrated care for pregnant women with substance use disorders. This at-risk population requires the clinician to be skilled in motivational interviewing, ASAM criteria, relapse prevention and coordinating care planning with the internal primary care treatment team, including peer support, as well as community agencies. All professional services are billed. Project Nurture positions are located in primary care and OBGYN clinics.
