Wekiva Springs Center is seeking a dynamic and talented Full-Time Case Manager to join our team of compassionate, dedicated professionals. Purpose: To provide provide patient care under the direction of The Director of Social Services for adults and geriatric patients with psychiatric and substance abuse conditions, and is responsible for using conducting ensuring coordination of care, and the arrangement of proper placement upon discharge.Â Must be able to handle effective a sizeable group of turnover in caseload.Â The Case Manager is also responsible for continuing his/her own professional development to enhance practice and the profession as a Case Manager. Actively participates as a member of the multi-disciplinary team of the unit and performs duties as assigned.Â This is a temporaryÂ position.Â They would manage the inpatient caseload andÂ identify the appropriate resources for them and make arrangements for a proper discharge.Â
