FOUNDATIONS RECOVERY NETWORK: What we are looking for: - Foundations Recovery Network is currently seeking a Recovery Care Coordinator to provide an integrated continuum of care between UHS/FRN Psychiatric and Substance Use Disorder (SUD) Treatment Programs through patient assessment, engagement, transfer systems, reciprocal referral management, and on site SUD education. We are looking for someone with the ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment. Key Responsibilities include: - Provides SUD marketing materials to patients identified with SUD and completes brief assessment of SUD history.
- Engages identified patients in a discussion about SUD treatment as part of their ongoing continuum of care.
- Obtains patient demographic, contact, and insurance information.
- Coordinates with Admission Center to complete clinical screening and verify benefits
- Uses MI to gain patient's commitment to SUD treatment.
- Participates in Clinical Pass Downs and Meetings at hospital to coordinate SUD aftercare treatment for patients identified.
- Upon admission approvals, coordinates transfer and admission to SUD Program, including arranging transportation.
- Provides SUD Education groups to patients, IP and OP locations, as requested.
- Responds to patient inquiries about SUD treatment.
- Provides weekly check-ins with patients transferred from hospital to SUD.
- Provides individual SUD training for UHS hospital case managers and program staff.
- Coordinates reciprocal referrals between UHS Psychiatric, and SUD facilities, including communication with family, referent, case manager, attending physician, and admission center.
- Completes all required paper and electronic chart documentation ensuring accuracy and confidentiality of information.
- Acts as role model by following program and treatment norms, demonstrating honesty and a willingness to listen to clients' issues and taking responsibility for own actions.
- Travels monthly to SUD Treatment Program for supervision
Knowledge, Skills and Abilities - Ability to inspire patient confidence in the UHS Staff and the integrated system of care.
- Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients, family members, referents, and co-workers through the integrated system of care.
- Excellent verbal and written communication skills required.
- Must have high level of interpersonal and customer service skills. Position continually requires demonstrated professionalism, tact and diplomacy.
- Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.
- Excellent organizational skills with the ability to prioritize workload and meet deadlines.
- Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, PowerPoint). Proficiency in MS Access (or other database program) is preferred.
- Understands need for and maintains appropriate confidentiality at all times when interacting with clients, residents, families, visitors, referral sources and all other contacts.
- Knowledge of psychiatric disorders and assessment for co-occurring disorders.
- Knowledge of addiction, addictive process and recovery.
- Knowledge of group process, group facilitation, and conflict resolution.
- A current, valid driver's license is required.
