The Medical Assistant Navigator reports to the Manager of Quality Initiatives and Measurement; will function across various settings as a liaison between patients and members of the healthcare team. In collaboration with the patient’s care team, the Medical Assistant Navigator is responsible for telephonic and/or face-to-face conversations with the patient concerning overdue screenings, obtaining external results and abstraction of information into the EMR. The Medical Assistant Navigator will assist patients in navigating healthcare systems, while providing support through phone calls as well asperform pre-visit planning functions to prepare the patient, and the healthcare team ahead of the visit.
Review practice site schedule 2 weeks ahead of visit and identify gaps in care
Contact patient concerning outstanding orders to ensure labs/screenings/procedures are completed
Contact external providers of care to obtain information and abstract into EMR so record is complete and up to date for visit
Assist patient with scheduling tests and/or visit
High School Diploma or GED required.
Graduate of an accredited Medical Assisting Program OR a Certified or Registered MA required.
Basic Life Support Certification Required or must obtain BLS Certification within 6 months of employment date.
KNOWLEDGE, SKILLS, & ABILITIES:
Excellent oral communication skills required.
Computer skills required.
Comfortable with calling patients to discuss care needs
Coordinates work with others
Demonstrates critical thought process and can work independently
Makes decisions based on policy and/or consultation with co-workers and team members
Requires listening skills to maintain cooperative associations
Proficient in Microsoft Office products
Associates Degree or equivalent, outpatient experience, certified or registered Medical Assistant preferred.
One year experience working within Family Medicine or Internal Medicine a plus.
