This position works within the Banner University Health Plan Complete Care program and is the single point of contact for information specific to the incarceration status of eligible members, coordination of incarcerated members and their treatment, oversight of contracted jail liaison and communicates jail related follow-up/requirements to Banner University Family Care staff, contracted providers, and Neighborhood Networks. This position serves as the Liaison between the jail system and the health plan in order to ensure coordinated jail releases.
Functions as the single point of contact to facilitate and coordinate the operational aspects of jail releases for health plans assigned members. Maintains strong relationship with and has a regular presence at county jails, criminal justice system stakeholders (primarily probation), and treatment facilities that provide care to member base.
Works closely with system stakeholders and advocates to develop and implement Collaborative Protocols to outline the interface between jails, health plan, and providers.
Coordinates access to services for members not previously seen by medical or mental health provider.
Participates in treatment assessments for determinations of level of appropriate care for Banner University Family Care members who are incarcerated within a county jail.
Work closely with Court Liaison to address gaps in access to services, smooth transition from jail to meeting court related responsibilities, and foster coordination with justice stakeholders and providers.
Ensures that assessments are as accurate and detailed as can be reasonably expected, and to assist members in navigating criminal justice, social service, and healthcare delivery systems.
Serves as a resource and provides leadership by attending meetings with AHCCCS and represents the health plan in all jail system related activities.
Provides technical assistance to health plan staff. Reviews and analyzes jail data, writes reports and prepares for regulatory reviews to ensure program compliance and ongoing performance improvement. Works with health plan managed care functions, Network IT, medical management, member services/engagement, contracts and compliance to develop, manage and submit any assigned regulatory deliverables to AHCCCS.
Knowledge, skills and abilities as normally obtained through the completion of a Bachelors degree in health care, social services or criminal justice administration.
Must have working knowledge of jail system and healthcare delivery system. Prefer three years experience in social services, preferably mental health experience, or an equivalent combination of experience and training. Has knowledge of Arizona Jail System, legal terminology, and criminal and civil court processes and procedures. Has knowledge of Arizonas safety-net healthcare delivery system. Requires excellent leadership skills and the ability to interact well across facilities, departments and organizations. Must have demonstrated ability to administrate high level meetings with a variety of stakeholders. Must be able to analyze and solve problems. Ability to communicate effectively, both orally and in writing. Ability to correspond and collaborate with managed care staff, court system stakeholders, and medical and behavioral health providers. Experience with developing, planning and implementing short and long-range goals.
Must be proficient on a computer (PC) with Microsoft Office products. Ability to work with databases/programs and applications.
