Your talent is to listen, strategize and deliver service based on the individual needs and preferences of your patients. And, you appreciate the need to focus on promoting long-term healthy eating habits through nutrition assessments, medical nutrition therapy, obesity and failure to thrive evaluations and diabetes self-management education that meet the nutrition, medical, and cultural needs of our patients.
Working in conjunction with the department of Pediatric and Family Medicine, the successful candidate will:
Be responsible for the nutritional evaluation, assessment, intervention, monitoring, and management of the pediatric population including their families
Perform comprehensive assessment of the nutritional education needs of patients enrolled in Diabetes Education Care in the Children’s Health Center and Family Practice on an individual basis and group sessions
Create personalized nutrition plans based on need’s assessment that promotes a healthy lifestyle
Offer nutritional counseling and serve as a resource for patients, families, and/or caregivers regarding the patients’ overall nutritional plan of care
Assist in planning, implementation and evaluation of the objectives and curriculum related to nutrition and diabetes
Effectively provide group nutrition education classes focused on obesity and failure to thrive, diabetes and other chronic conditions
Participate in professional meetings and conferences
We are looking for someone who has:
A Bachelor’s Degree in Nutrition/Dietetic
Minimum 3 years of progressively responsible, related experience in a similar program
New York State License as a Certified Dietitian-Nutritionist (CDN)
Registration with the Commission on Dietetic Registration (CDR)
Sensitivity to cultural diversity of patient population served
Strong verbal and written communication skills
The ability to work well with physicians, nursing and ancillary staff
