Utilizes clinical judgment and expertise to coordinate care management activities for adult pulmonary patients.
The primary responsibilities of this position are to perform patient assessment, develop and implement treatment plans, provide education to both patients and their families, and complete discharge bundles prior to discharge and ensure all components of transitions of care are complete.
This individual will work closely with the pulmonary team through rounding and daily communication.
This individual will serve as a liaison between pulmonary medicine and the inpatient respiratory therapist.
Facilitates seamless care for adult pulmonary patient, both outpatient and inpatient, and their families, and ensures effective and timely use of resources to address identified needs.
Promotes quality care delivery across the continuum of care.
Performs all duties to the highest level of patient care and maximum patient satisfaction, in accordance with the goals and objectives of the Geisinger Health System.
Works under the general direction of the assigned providers and the Operations Manager.

EDUCATION AND/OR EXPERIENCE:
RRT with current Pennsylvania licensure required and NBRC and AARC credentials. (COPD Educator certification)
Requires a minimum of three years of related clinical experience.
BLS, ACLS, and PALS certified or eligible to be certified, EPIC experience preferred, willing to obtain AE-C.
Bachelor of Science.
