Incumbent is responsible for assessing a Veteran's functional needs on and off site thru face-to-face, virtual and in home visits. Incumbent works with Program Coordinator(s) to complete assessments, re-assessments of function, mobility and safety. The Clinical Assessor conducts interventions to identify caregiver stress, psychiatric illness, emotional distress and social needs. Incumbent serves as a consultant for the processes caregivers, Veterans, and families encounter with outside agencies.
