As a Social Worker Program Coordinator assigned primarily to the Women Veterans Program (WVP), you will provide clinical and administrative support in the implementation of services for women Veterans. The coordinator assists in the execution of comprehensive planning for women's health issues that improve the overall quality of care. The WVP Social Worker Coordinator demonstrates integration of biopsychosocial concepts, cognitive skills and technically competent SW practice. The Women's Health Social Work Program Coordinator's primary responsibilities are to develop rigorous community outreach systems to identify and engage Women Veterans into comprehensive programming and to determine and implement those evidence based medical social work services and interventions to be provided in the continuum of Women's Health Specialty care. Incumbent will be responsible to develop a multi-faceted approach working in diverse geographic catchment areas to outreach and engage Women Veterans. The geographical challenges of VAPAHCS will be addressed by the incumbent, who will develop an outreach program utilizing telehealth to address specific medical and psychosocial needs of remote female veterans, coordinating care with primary care providers. The incumbent assists in the execution of comprehensive planning for women's health issues that improve the overall quality of care provided to Women Veterans. The WVP Social Worker identifies and implements integration of biopsychosocial concepts, cognitive skills and technically competent Social Work practice in providing coordination of gender-specific services to Women Veterans. Social Worker is to assist in coordinating the provision of comprehensive health care services to women veterans; medical and/or surgical health care services and psychosocial care of their physical and mental health needs. Responsibilities also include resource development; continuous quality improvement activities; and evaluation/consultation. In addition, the incumbent must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations. The Women's Veteran Program Social Worker Coordinator is responsible to work closely with the Women's Veteran Program Manager to develop and implement an aggressive outreach program to identify and locate Women Veterans in need of health care services. The incumbent works collaboratively with medicine, mental health and specialty clinics in planning, organizing, managing and evaluating the social work services needed to further the mission of the clinic in providing exceptional health care to improve the health and well-being of the veterans served by the clinic. The incumbent serves as a primary liaison between the VA and community agencies. Develops Women's Health outreach events and activities in the local community and state to promote and recognize Veteran health concerns. Participates on state and local Women's Health specific committees. Invites state and community Women's Health related agencies to participate on the local VA Women's Health Advisory Board and other specialty consultation boards. This is 1.0 FTE NTE 1-year position due to grant funding with potential for reapplication. Work Schedule: Monday through Friday 8:00 AM to 4:30PM Financial Disclosure Report: Not required Telework, if approved, would be on a regularly scheduled basis during the week, but this is not a virtual or remote position.
