The incumbent is an Advanced Licensed Social Worker assigned to the Chief of Social Work or designee. Works as a Senior Social Worker (Team Lead) in the Athens HUD-VASH program. Provides oversight, technical and clinical guidance to social work staff in the Athens HUD-VASH program. Must have experience that demonstrates clinical and leadership skills. Major duties include, but are not limited to the following: Functions as the team lead for the Athens HUD-VASH program. Provides oversight, technical and clinical guidance to social work staff in the Athens HUD-VASH program. Evaluates relevant issues to the supervisor as indicated. Responsible for compiling reports, summaries, and other data as requested by leadership. Assist in the ongoing preparation for CARF accreditation. Provides comprehensive psychosocial services to Homeless Veterans enrolled in the HUD-VASH program. Performs outreach for Homeless Veterans throughout the local Athens and surrounding areas and work with internal and external stakeholders. Function independently and will identify and screen patients within the HUD-VASH program for psychosocial needs in an effort to prevent loss of housing and/or hospitalization. Spends the majority of the workday in the community and will be required to operate a government vehicle. Work Schedule: Monday - Friday, 8:00a - 4:30p (The individual may be required to work an irregular tour of duty, to include weekends depending on the needs of the service and medical center. The individual will participate in the medical center's social work on-call rotation, as required. Social work on-call coverage is between 4:30p - 8:00a, to include weekends and holidays).
