This Social Worker position is located at the Edward Hines Jr. VA Hospital in Hines, IL and is part of the VA's Great Lakes Health Care System in the Veteran's Integrated Service Network (VISN 12). The incumbent in this social work position is assigned to the Transition and Care Management Program, a Social Work program within the Mental Health Service Line (MHSL).Positions can be located throughout the medical center, including: Medical/Surgical Units, Mental Health, Substance Abuse, Primary Care, Extended and Community Care, Specialty Care as well as program specific responsibilities along with general duties. Major Duties: Requires the ability to conduct psychosocial assessments of a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds. Basic knowledge of psychosocial treatment modalities and under supervision, ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Has working knowledge and skill in the use of word processing, data management, and other computer software. Ability to learn and utilize software programs in use by the VHA. Serves as liaison to community agencies and networks with agency representatives. Effectively communicates with and utilizes community agencies to facilitate treatment, discharge planning, and continuity of care. Demonstrates an understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs and age associated illnesses and conditions. Verifying the veteran's status, eligibility for VA medical care, and eligibility for program participation. Implements and monitors OSHA goals for Life Safety Management, health and the environment procedures. Reports safety hazards, accidents and injuries. Coordinates care with interdisciplinary team to promote continuity Post-9/11 service members and/or Veterans and their families. The incumbent reports programmatically to the facility Transition and Care Management (OEF/OIF/OND) Program Manager, who assigns severely ill and/or injured Post-9/11 Veterans to the social worker case manager's panel. Supervisory consultation is provided based on the need of the individual social worker. Work Schedule: Monday through Friday, 8:00am to 4:30pm. Financial Disclosure Report: Not required
