The Behavioral Health Interdisciplinary Program (BHIP) Social Worker/ Psychotherapist is a part of a multidisciplinary team of health care professionals and administrative staff that provide comprehensive accessible patient centered mental health care. The team manages, coordinates, and provides Veteran-centered, recovery-oriented, and evidence-based care that is consistent with the Veteran's identified treatment goals. Assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence Acting as an advocate with appropriate VA and community service provides and agencies Assessing the psychosocial functioning and needs of Veterans and their family members, identifying the Veteran's strengths, weaknesses, coping skills, and psychosocial acuity Collaborating with Veteran, family/caregiver, and interdisciplinary treatment team to facilitate the delivery of health care services Conducting timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plans as appropriate Providing a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy Formulating and implementing treatment plans including measuring achievable goals, and identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity Establishing and maintaining ongoing education programs for Veterans, community agencies, students, and staff Providing consultation to professional staff within SAVAHCS and to community providers, concerning clinical assessment findings and appropriate treatment plans for patients Developing, implementing, and documenting the psychotherapeutic treatment plan for an assigned caseload of patients, including prompt completion of clinical reminders Work Schedule: Monday through Friday, 8:00 am - 4:30 pm, with one evening tour of duty per pay period.
