The social worker serves as a member of a multidisciplinary Housing and Urban Development Veterans Affairs Supportive Housing (HUD VASH) team and is responsible for coordinating and providing individual and family counseling and case management services. The social worker; conducts assessment interviews, develops treatment plans and provides treatment to HUD VASH eligible Veterans and their families. Duties include, but are not limited to: Provides HUD-VASH case management Performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes. Verifies the veteran's status, eligibility for VA medical care, and eligibility or program participation and is responsible for ensuring the homeless veterans are considered as primary participants for program acceptance. Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery and linking and referring veterans to the VA Medical Center, VA Regional Officer and/or community agencies. Actively participates in the HUD-VASH Program Conference calls and attends training offered by the HUD-VASH program and is responsible following local policy and SOP's for the HUD-VASH Program. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for an making recommendations on program procedure or policy changes based upon work experiences. Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. Works within the VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH program veterans.
