The Social Worker serves as the primary facilitator for admissions into the VA Bedford Healthcare System Domiciliary (Mental Health Residential Rehabilitation Treatment Program), providing comprehensive care to a veteran population with history of homelessness, substance abuse and complex mental health needs. screens and assesses veterans to establish facts about the veterans situation, presenting problems, and the impact of problems on the health of the veteran; evaluates each assigned veteran's situation, including the veteran's reaction and ability to deal with current stressors, and arrives at a reasoned conclusion; develops treatment plans, with the input of the veteran and the interdisciplinary team; provides independent consultation and makes recommendations; evaluates progress towards goas and adjusts treatments plans as appropriate; and, provides guidance, advice, emotional support and other assistance as needed by the veteran.
