VA Connecticut Healthcare System is seeking one full-time National Telehealth Program Staff Senior Social Worker located at the Yale West Haven campus in Orange, CT. The VISN Clinical Resource Hub (CRH) Suicide Prevention National Telehealth Program Staff position is administratively in the VACHS Mental Health Care Line and provides specialty clinical services throughout VISN 1. Duties include but are not limited to: Provides a wide range of care including risk assessment and referral, intake/diagnostic and psychosocial assessments, comprehensive treatment planning, and evidence-based psychotherapy services in a manner consistent with established protocols to Veterans at risk for suicide. Engage in other suicide prevention related tasks and needs, such as training, consulting, and future suicide prevention initiatives. Collaborates with Substance Abuse Treatment Program, Suicide Prevention facility-based teams, and other Behavioral Health services to ensure a seamless transition to appropriate services for Veterans. Providing clinical services in a virtual modality is expected. Coordinates virtual modality that includes video telehealth, patient/provider secure messaging, telephone visits, and review of electronic medical records. Work Schedule: Monday-Friday. Hours: 8:00am-4:30pm. Possibility of irregular tours and/or weekend tours. 100% Remote available. Financial Disclosure Report: Not required.
