This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. The full time permanent GS-12 Social Worker (Program Coordinator) serves in the Ambulatory Care Service Line at the Reno, NV location of the VA Sierra Nevada Health Care System. The Caregiver Support Program Coordinator (CSPC) is primarily responsible to administer, develop, and implement the Caregiver Support Program. He/She is responsible to make program assignments to other Caregiver Support Program team members and insures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives. The CSPC administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans. The CSPC develops and implements information systems to track service activities including visits, referrals, caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. The CSPC develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services. The CSPC assists in assessment of caregiver stress/burden and assists in the development and application of directed clinical treatment interventions if necessary. The CSPC advises and collaborates with other interdisciplinary teams throughout the medical center on caregiver issues. The CSPC creates educational tools, develops programs and implements training focused on specific caregiver needs/issues. The CSPC also provides education and in-service training to VAMC employees, Veterans, families and caregivers. The CSPC develops and facilitates caregiver support groups (face-to-face or VANTS telephone line) and educational programs. The CSPC assists in the oversight of distributing educational materials provided by VA, local, state and national caregiver organizations to VAMC employees, Veterans, and families and caregivers. The CSPC assists in conducting in conducting caregiver and family clinical interventions to facilitate the identification of caregiver stress, psychiatric illness, emotional distress and social needs as treatment issues. The CSPC uses professional judgment, including the knowledge of normal and abnormal behaviors, as an inherent competency applied in daily interactions with caregivers and the Veterans they care for. The CSPC will be the primary point of contact and provide oversight for the Veteran Eligibility Assessment Clinic, to include the coordination of Veteran / Caregiver appointments for both eligibility assessments. The CSPC will collaborate with the interdisciplinary teams in Primary Care, Medicine, Behavioral Health and other Services as needed to ensure that clinic operations are maintained. The incumbent will conduct home visits in the community and in rural Northern Nevada and California.
