The social worker communicates with team members the functions they are responsible; they initiate discussions with Veterans/Caregiver to involve them in shared decision process to assist with the Veterans healthcare, including advanced care planning; attend discipline specific, program, unit administrative and clinical meetings or in-services; they participate on assigned facility, program, discipline specific service committees or system redesign processes. Grant and Per Diem Program (GPD) Liaison: - Providing services to, and oversight of, the GPD-funded community-based programs as outlined in this Handbook and the GPO regulations. - Verifying the veteran status and eligibility of program participants. - Verifying admission and discharge dates of program participants for billing purposes. - Collecting and submitting GPD-funded program participant data as outlined by NEPEC evaluation procedures. - Providing oversight of GPD-funded program participants' care and, at times, case management of those participants at sites where organizations are receiving per diem payments. Case management is defined as the provision of services by VA clinical staff to homeless veterans, which include Monitoring the care in, and assessing the compliance of the program receiving per diem as outlined in the recipient's original grant applications; Intervening to facilitate compliance, or correction, of the program, when appropriate; liaison will perform a cursory environmental review and identify any obvious hazards or other deficiencies, which need to be corrected; - Conduct clinical and environmental inspections are to ensure grant compliance. Primarily, inspections function to ensure all activities put forth in the GPD-funded program's original application are carried out as prescribed in the proposal. - Case Management. Liaisons are expected to provide case management, when necessary, at sites where organizations are receiving per diem payments. For these purposes, case management is defined as provision of services by VA professional staff to homeless Veterans. - Length of Stay and Extensions. Length of stay in transitional housing programs is limited to 24 months. Extension requests are reviewed and documented. Liaison meets with Veterans and community providers to assure each Veterans has discharge plan to meet goals of the program. - GPD Liaisons must ensure that community provider and their clinical documentation must be in compliance with local VA medical center policy and procedures, including admission notes, reassessment progress review notes and discharge notes - Monitor and report is annually on funded community agencies providing services to homeless Veterans. The purpose is to promote the development and provision of transitional housing and supportive services with the goal of helping homeless Veterans achieve residential stability. - Meet weekly with funded GPD program staff and provide oversight to assure Veterans admitted increase their skill levels and/or income, and obtain greater self-determination. Provide mentoring and oversight to Peer support staff assigned to teams; meeting and documenting per policy or procedures. - Provide unit/program orientation and guidance for specific functions to assure the team members are compliant and meeting program objectives. - Participate in program planning and review for GARF and other accreditations for continued readiness. - Provide first level supervision or point of contact as requested or designated in the immediate supervisor absence for service recovery. - Monitor process and participate in system redesign as needed or requested with the goal to streamline and meet outcomes. - Provide and assure services for Veterans are assigned through community case management and document the overall level and effectiveness of the community services provided utilizing patient centered care approach to assure Veteran is involved in decisions regarding their healthcare. - Collecting and submitting participant data as outlined by VA program procedures.
