The VA Southern Nevada Healthcare System is recruiting for a Senior Social Worker to provide clinical case management and care coordination in the Dialysis Clinic. This position is under the Social Work Service care line located in Las Vegas Nevada. Interviews Clients and Conducts Psychosocial Assessments Interviews Veterans to establish facts about the Veteran's situation, presenting problems and the impact of such problems on the Veteran's functioning and health. Reviews all referral data from social workers, case managers, managed care staff and discharge planners to identify patients' needs. Makes a clinical assessment identifying the Veteran's needs and strengths. Assesses high risk factors, acuity, and need for VHA services and community services. Makes Psychosocial Diagnoses Independently evaluates the Veteran's situation, including the Veteran's reaction and ability to deal with it, and arrives at a reasoned conclusion. Assesses at-risk factors and develops a preliminary disposition plan involving the Veteran and family or significant others. On an inpatient/ outpatient basis, finds a suitable means of treatment to help Veterans and/or significant others copes with stressful situations. Evaluates his/her practice on an on-going basis through participation in professional peer review, case conferences, research studies, or other organized means. Plans Effective Treatment Assures the transfer of health care, both inpatient and outpatient to the appropriate VHA and community resources. Is responsible for clinical social work practice in the area or program to which assigned, including promotion of Social Work services as a key component of treatment. Makes adjustments to the psychosocial treatment plan and interventions based on changing needs and response to interventions. Implements Treatment Provides clinical services to Veterans and family members/ significant others in support of the Veteran's treatment. Provides individual and group counseling services, including psychotherapy in some settings. Regularly treats Veterans with serious medical and psychiatric problems through counseling or referral. Demonstrates acceptance, interest, encouragement, and selective use of questioning, explanation, and guidance in working with Veterans to help them explore their problems. Receives and completes requests for services (consults) for interdisciplinary team members and from other Social Workers on complex, difficult cases, using advanced practice skills and expertise. Evaluation, Education, Supervision, Coaching, and Mentoring Establishes a continuing relationship with the Veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate. Meets with Veteran to assess accomplishments and re-establish goals. Modifies services to best meet the treatment needs of Veterans and to promote efficient practice. Coordinates Social Work services with other services offered in the treatment program assuring such services are complementary and comprehensive. Explains Veteran's treatment and progress to third parties as appropriate. Manages the quality control and evaluation of the professional services provided. Provides clinical supervision to lesser experienced social worker staff members Outreach Required to conduct active outreach to Veterans involved with the medical center and local community. Comprehensive Case Management Functions as an ongoing case manager by performing duties to facilitate independent community living by coordinating mental health and community care, as appropriate. Program Coordination Actively participates in the program conference calls and attends program trainings and meetings. Responsible for following local policies and procedures for all social work and mental health programs. Responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested Maintains productivity at the established required level Coordinate treatment and communicate with the care lines to facilitate continuity of care for Veterans Document social work interventions and activities in patient's clinical record and ensure appropriate hand off when transferring of patient care At times may participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends Travels where necessary, or as assigned in the capacity of community outreach support. The incumbent will maintain a current, unrestricted driver's license Acts as a liaison between VA and community resources, to include marketing and public relations duties as needed Performs other related duties as assigned
