This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The VA Northern California Health Care System Social Work Service is currently recruiting for a Senior Social Worker. This position is located at the Mather VA Medical Center. The Homeless Outreach Coordinator coordinates coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. The Homeless Outreach Coordinator serves as the VA facility primary point of contact (POC) on the local Stand-Downs, participating on the planning committee and serves as key chairperson on Stand-Down subcommittees. POC assist in coordinating the involvement of other VA staff members and outlining their roles in supporting Stand Downs. The Senior Social Worker provides outreach, identifying and screening patients for psychosocial needs and provides ongoing case management and follow-up. The Homeless Outreach Coordinator provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. The Senior Social Worker establishes and maintains effective therapeutic relationships with veterans and their families. The Senior Social Worker may provide support to other programs to include but are not limited to; supporting the HUD-VASH program by managing a caseload, assisting in Veterans Justice Outreach activities, POC for National Call Center for Homeless Veterans, as well assisting in a Homeless-Patient aligned care team.
