This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The VA Northern California Health Care System is currently recruiting for a Senior Social Worker for the Community Living Center (CLC) at the Martinez location. The Community Living Center (CLC) Senior Social Worker may be assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns. Assignments include clinical settings where they have limited access to onsite supervision. The senior social worker collaborates with the other members of the treatment team on site in the provision of comprehensive health care services to Veterans, ensures equity of access, service, and benefits to this population, ensures the care provided is of the highest quality. The senior social worker provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for other staff. They serve on committees, work groups, and task forces at the facility, VISN and national level, or in the community as deemed appropriate by the supervisor, or GEC Chief of Staff. The senior social worker initiates and conducts a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They identify areas for improvement, gathering relevant data, assessing the data, developing, and implementing ideas for improvement and evaluating efficacy of improvement efforts. Major duties include, but are not limited to: Participate in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the CLC. Educate medical center colleagues and/or students regarding the various GEC programs and services and assist with dissemination of information to other VA facilities and community partners. Ensure that a comprehensive CLC care data tracker is always up to date. Identify and develop program resources in the community, providing liaison with these providers. Complete an in person initial screening and evaluation of Veterans to determine their needs and if they meet criteria for inclusion in various GEC programs in order to maximize function and independent living to the extent possible. Develop a treatment plan of the Veteran in collaboration with the referring and CLC interdisciplinary team, the Veteran, family members, and caregivers, whenever possible. Serve as a liaison between the CLC Team and specialists and Veteran and/or family. Collaborate with other service providers in reassessing the Veteran's needs for non-institutional and/or institutional services and programs and entitlements. Review services which may be used to provide in home services to Veterans. Make rapid assessments and developing crisis management plans to maintain a patient in the home or for admission to acute, short term, and/or long-term placements. Provides education related to VA and community resources, entitlements, Advance Directives/Living Will, Life-Sustaining Treatment decisions, and will refer Veterans and/or families to other GEC programs or appropriate VA resource for identified health education needs.
