The incumbent functions independently as a Case Manager for the Homeless Veterans Program's HUD-VASH Program, a collaborative effort supported through rental assistance vouchers and VA's services. The incumbent is responsible for program implementation and ongoing contact with local Public Housing Agencies and providing case management-based services to ensure that veteran care is coordinated with the VA, community health or mental health providers, and agencies that serve homeless population. A RELOCATION INCENTIVE HAS BEEN APPROVED FOR THIS POSITION AND IS AUTHORIZED FOR HIGHLY QUALIFIED CANDIDATES.
