The VA Pacific Islands Health Care System is recruiting for a Social Worker, Home Based Primary Care (HBPC) in Kona, HI. The mission of the HBPC program is to provide intensive, community-based case management to veteran's who are frail, homebound, and have multiple chronic medical issues. The HBPC Social Worker independently completes psychosocial assessments and provides social work services to approximately 70 HBPC veterans on Kauai. In addition, the HBPC Social Worker provides assistance in meeting the psychosocial needs of veterans enrolled in the HBPC program.
