Position Summary:Position will work in Palliative Care, scheduled 2 8-hour days week 1, and 3 8-hour days week 2.Manage the medically related social and emotional needs of patients and their families as needs impinge on patient medical conditions, treatment, recovery and safe transition from one level of care to another.
