Multicare Rockwood Clinic is seeking a Licensed Clinical Social Worker who is passionate about patient care and committed to clinical excellence. The position is co-located in a primary care clinic, giving you the opportunity to work closely with primary care providers to improve patient health outcomes through quality care and integrated consultations. As a growing behavioral health team, now is the perfect time to join us!While working as part of a multidisciplinary treatment team, the LICSW works in collaboration with other clinical staff and community support providers to provide high quality integrated services. The LICSW functions autonomously in providing therapy services to patients with a variety of acute and chronic mental health conditions. Expectations include conducting individual assessments, including diagnosis, individual treatment planning, and referrals as appropriate and providing individual, family, and group behavioral health therapy services in a primary care setting. The nature of the services necessitates a short-term model of care. The LICSW must work collaboratively to provide evidenced-based practice with the primary care team. Other expectations include completion of clinical progress notes in a timely manner, effectively communicating with providers, patients, and family members, providing clinical consultation and educational programs to primary care personnel as needed, and attending regularly scheduled case consultation and department team meetings with behavioral health and primary care providers.Qualifications:
Master's Degree in Social Work, Clinical Psychology or closely related field LICSW, LMHC, LMFT (Must be independently licensed)
Knowledge in crisis intervention techniques and short-term therapy
Ability to work cooperatively with a multi-disciplinary team.
Strong case conceptualization skills
Experience facilitating individual, group, and family therapy sessions preferred
Ability to build rapport and establish a strong therapeutic working relationship with patients
Experience providing evaluation and evidence-based psychotherapy with a broad spectrum of clinical presentations preferred
Excellent written and verbal communication skills
