Overview
Atlantic Health System is seeking a Full-time Social Worker for the Neuroscience Department.
Responsibilities
Works with Health Care Team in identifying outpatients and family/significant others psychosocial, financial and emotional needs.
Receives direct referrals from physicians, nurses, navigators.
Completes Psychosocial Assessment and develops plan for needs.
Collaborates and communicates with Health Care Team to reassess status on an ongoing basis.
Provides information to team members regarding community resources and patient/family follow through:
* Implement and facilitate a plan to address patient/family needs by providing supportive information. Patients will receive a new patient packet that includes programs (internal and external)/support groups, contact information.
* Coordinate patient/family care through visit or phone visit, providing grief counseling, medical equipment set up, mental health assistance, visiting arrangements (hotel), legal services, financial planning and resources. Connect patients with the community as needed: pharmacy assistance, meals on wheels, interventions, etc.
* Collaborates with corresponding departments such as Palliative Care, pt. financial services patient experience, to provide best measures to assist patient/family individually. Provide coping mechanisms to family's and assist the family to navigate through meeting personal goals, advanced directives, living wills and provided good quality of life; ex: assist family writing letters to family members, or making videos.
* Coordinate the resolution process by developing a calendar of events for follow up (follow up care appointments) and Provides consistent, clear communication to referring team members on patient progress/resolution.
* Creating a binder of resources for NeuroOncology and Neuroscience support services, as well as, sub-department specific folders with resources information and pamphlets for specific areas.
* Social work assessments and documentation are completed following patient/family communication and located in the electronic medical record.
* Provides support as needed by phone or during office visits to identified patients/caregivers.
* Facilitates team support of identified patients and family needs through on-going communication with interdisciplinary team (physicians, nurse practitioners, medical assistants, etc.).
* Documents as appropriate in electronic medical record system (EPIC).
* Maintains working knowledge of community resources, such as housing options, home care, adult day services, social service programs, insurance, transportation, etc.
* Facilitates referrals to appropriate agencies as needed, such as adult protective services, Senior Support Program, etc.
* Assists as needed for support services programs (Healthy Aging Program, caregiver education series, etc.).
*Develop education symposia and support groups where applicable.
* Support patient needs by navigating pts through their journey and supporting their psychosocial needs.

* Physical Requirements: As defined by Occupational Safety/OMS.
Qualifications
Education:
MSW required
LSW or LCSW preferred
Relevant Experience:
At least 2 year related experience perferred
