This Social Worker is assigned to Lebanon Primary Care, Patient Aligned Care Team (PACT), Specialty and Spinal Cord Injury Clinics with responsibility to provide social work services to adult and geriatric Veterans. Provides open access and immediate response to Veterans in psychosocial crisis in the Lebanon Primary Care and Specialty Clinics to include supporting their behavioral health needs.
