Home Based Program Coordinator (HBPC) is an extended care program at the New Mexico VA Health Care System in the Geriatrics Extended Care Line (GEC) that uses an interdisciplinary team, working with primary care providers to deliver healthcare services to patients in their homes, in concert with family and community support. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
