This Social Worker is assigned to VA Community Clinic Cumberland, Patient Aligned Care Team (PACT) & Specialty Clinics with responsibility to provide social work services to adult & geriatric Veterans receiving care in at the Lebanon VAMC. This Social Worker will provide open access to Veterans seeking these services & immediate response to individuals in psychosocial crisis in the Lebanon Primary Care & Specialty Clinics, as well as supporting behavioral health needs of those in crisis. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
