The Health Care for Homeless Veterans (HCHV) Outreach Social Worker provides triage services, psychiatric assessment, referrals and support for homeless Veterans seeking services from Community Integration Services and in community based shelters, soup kitchens and places where homeless Veterans may congregate. The HCHV Outreach Social Worker will locate Veterans in the community and engage Veterans seeking services through Community Integration Services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
