Vanderbilt University Medical Center Temp Force is accepting applications for inpatient Social Workers within our Transition Management Office (TMO). The Social Worker provides psychosocial assessment of and brief targeted intervention for social, emotional and environmental issues that impact client/family's ability to optimally benefit from care. Actively collaborates with team to meet patient care management and system improvement goals. Represents the Social Work department on institutional and community committees/task forces. Provides leadership for department committees/workgroups. Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs. New grads are welcome to apply!
Vanderbilt University Medical Center TempForce (TempForce) was created to connect temporary talent to all areas of VUMC. TempForce places diverse and skilled personnel to departments across the Medical Center. In any given year, TempForce fills a wide variety of temporary requests including pharmacists, administrative/clerical roles, medical assistants and more.
Transition management or commonly referred to as care coordination is a thriving department comprised of nurses, social workers, and support personnel. Currently there are 150 staff however the department has increased their staff needs to enhance the patient experience at Vanderbilt. The Vice President of TMO is Marcia Colone, Ph.D. who has over 25 years of experience in care management. Dr Colone leads a team of experienced Directors and Team Leads who mentor, coach and support staff within the coordination team.
