The incumbent works closely with our Community Care Team, which includes: Case Managers (RN), MSA/PSA, Administrative Program Manager, Nurse Manager, etc. The team falls under the Chief of Staff. The social worker in Community Care often functions as a care coordinator to ensure that health care meets the needs, as defined by the Veteran. *This position is anticipated to continue through 09/30/2023.
