This Social Work position is located at VA Western New York Healthcare System. The Social Worker (PACT) is assigned to Outpatient Primary Care, Patient Aligned Care Team (PACT), is a part of and works in collaboration with the extended Interdisciplinary PACT. The (PACT) Social Worker is responsible for delivery of care, referrals for treatment services and case management services. This position is eligible for telework, and serves two locations Durkirk and Jamestown.
