This position is a half time position located in the Mental Health Care Line, with primary assignment to the Home-Based Primary Care Program located within the Geriatric and Extended Care Line. The VA Home-Based Primary Care (HBPC) program provides comprehensive, interdisciplinary primary care services in the homes of Veterans with complex and chronic, disabling disease.
