English: Must be proficient in spoken and written English. Education: Must have a Master's degree in Social Work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure: Must be licensed or certified by a state at the advanced practice level at the GS-12 grade level. This includes an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (PROVIDE COPY) IN ADDITION TO THE BASIC REQUIREMENTS YOU MUST MEET THE FOLLOWING GS-12 GRADE REQUIREMENTS: GS-12 Grade level: Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: The work is primarily sedentary. Physical demands do not exceed those of a typical office setting. This position is responsible for assessing, planning, organizing, and coordinating all Palliative Care Services and programs at the VA IIliana Health Care System (VAIHCS) including inpatient Palliative Care Unit. The candidate also provides clinical Social Work services for patients on the Palliative Care Unit as well as on the Palliative Care Consult Team and outpatient Palliative Care Clinic Major Duties: Responsible for ensuring that all Palliative Care programs and services at VAIHCS comply with VHA and Medical Center policy; Federal, State and Local regulations and meet Joint Commission requirements. Responsible for collecting and maintaining statistical data, preparing reports and overseeing performance improvement activities related to Palliative Care programs and services. Develops new initiatives and refines present programs and services to meet the needs of Palliative Care patients in the face of ever-changing environment, care practices and new technology. Completes psychosocial assessments, develops treatment plans and provides counseling for patients/families on the Palliative Care Inpatient Unit and are referred to the Palliative Care Consult Team and Palliative Care Clinic. Provides other Social Work interventions including crisis intervention, advocacy and linkage to community resources. Provides individual, group and family treatment interventions as indicated to assist patients, family members and significant others coping and dealing with loss and grief experiences in relation to disability, terminal illness and death.
