This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j, this part. Grade Determinations: Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. ["The VA Northern California Health Care system is currently recruiting a Social Worker Program Coordinator for Medical Foster Home (MFH) program the Geriatrics and Extended Care service at the Redding location. The social worker will participate in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the MFH program. The social worker will educate medical center colleagues and/or students regarding the MFH program and assist with dissemination of information to other VA facilities and community partners. The social worker will evaluate MFH practices using relevant statutes and regulations and take action to improve identified deficiencies. The social worker is responsible for and will track MFH program assessments, consults, authorizations, statistics, data management, identify significant findings, problem solve, complete documentation reports pertaining to caseloads, productivity, and trainings. The incumbent must ensure that a comprehensive MFH data tracker is always up to date. The social worker will complete an initial screening of Veterans to determine whether they meet eligibility criteria for inclusion in the MFH program. The social worker will serve as a liaison between the contracted MFH provider and Veteran and/or family. The social worker is the subject matter expert on other VA and/or community resources. The social worker recruits prospective MFH caregivers and homes. Reviews MFH caregiver applications and determines if applicants are suitable to proceed based on requirements. Coordinates the pre-approval inspections of all prospective medical foster homes as well as the annual inspections of all approved medical foster homes, to include coordination with interdisciplinary inspection team members, conducting the on-site inspection, compiling the survey report, and following up on all corrective action plans. The social worker is responsible for reviewing VA and community resources which may be used to provide in-home services to Veterans. The social worker is also responsible for developing a resource file of VA and community social service programs and will refer the Veteran to needed services. The social worker provides education related to VA and community resources, entitlements, Advance Directives/Living Will, Life-Sustaining Treatment decisions, and will refer Veterans and/or families to other GEC programs or appropriate VA resource for identified health education needs.
