Accepting Applications:   8/29/2022 - 9/12/2022 Detailed Description Conducts assessments on identified at-risk individuals in the community, to include minors and adults, focused on threats of violence and mental health concerns.   Determines and implements plan of intervention, and coordinates referrals and delivery of services. Provides support, education and guidance to the referred individual and family. Manages an assigned caseload of referred individuals. Exercises independent clinical judgement and responsibility for cases. Evaluates individual’s level of functioning and status of engagement with services and their efficacy.
