The Syracuse VAMC has 1 vacancy for a HCHV Outreach LPMHC for the Healthcare for Homeless Veterans (HCHV) team. The incumbent performs counseling duties specifically directed toward chronically homeless Veterans and their families. They independently provides community based clinical, psychosocial and case management services for eligible Veterans and their family members.
