The HPACT Senior Social Worker provides services in the Homeless Patient Aligned Care Team clinic in Community Engagement and Reintegration (CERS) the clinic is located at the West Los Angeles Healthcare Center, a co-located integrated Homeless Patient-Aligned Care Team (HPACT). The HPACT clinic team strives to improve continuity of care and coordination of services for Veterans who are either homeless or at risk for homelessness by working with existing clinical and community care programs. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
