The Lead Post-Acute Care (PAC) Counselor assists the Director of Strategic Partnership in building awareness and volume of Post-Acute Care programs across the Integrated Delivery System (IDS).   In addition to the day-to-day duties of the PAC Counselor, the Lead will assist with PAC Counselor team management including operations and schedule management.  This position serves as the point of contact for the PAC Counselor team and will coordinate Post-Acute Care Services education efforts across the enterprise.  The Lead Counselor will develop and maintain oversight of referral and activity logs and help manage team workflows. Day-to-day PAC Counselor duties:  Coordinates referrals and admissions for the Post-Acute Care Services Division; develops and nurtures positive relationship
