The New Mexico VA Healthcare Network is recruiting for a Social Worker assigned to the Medical/Surgical area at the NMVAHCS and practices in the inpatient medicine units, providing coverage to inpatient surgery and other inpatient medical units. The Social Worker provides care continuity between the acute setting and community-based services achieved through case finding, and coordination of after-care needs based upon integration of medical and social factors in relation to available resources. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
