The Social Worker - Ambulatory Care Supervisor manages the administrative and clinical services of social workers assigned to Patient Aligned Care Teams (PACT). The incumbent is responsible for program coordination and supervision of PACT Social Workers. The incumbent manages responsibility for clinical and administrative operations at the John D.
