Overview

St. Luke's Health System in Boise is seeking a Social Worker (LSW) to join our Rehab team.

**This position works Monday-Friday

Responsibilities

• Interviews patients and relatives to obtain social history relevant to medical problems and planning.
• Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care.
• Serves as a member of a multi-disciplinary team and is a liaison with community resources.
• Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies.
• Interprets and assists in resolving social problems that relate to the medical condition and/or hospitalization.
• Documents assessments, plans and outcomes.
• Other duties and responsibilities as assigned.

Qualifications

• Education: Bachelors' Degree.
• Experience: None
• Licenses/Certifications: Licensed Social Worker (LSW) in state(s) of practice.
