SUMMARY OF POSITION:
Under the direction of an RN Care Coordinator (RNCC) or Masters prepared Social Worker (MSW) the purpose of this position is to assist in communicating the discharge plan to patients and families and facilitating safe and timely transitions of care and services from acute care to alternative levels such as skilled nursing facility, inpatient rehabilitation, and/ or home program. This role will work collaboratively with the care transitions team members, providing onsite face to face interactions with patients and families and telephonic discharge arrangements with external entities and post-acute vendors. This role will assist in facilitating timely discharges, identification of barriers, and assuring clear communication is provided to patient family regarding discharge and transition of care plan.
MINIMUM QUALIFICATION(S):
Bachelor's degree in social work, health and human services, or allied health field or equivalent experience (4 years)
BLS certified, or ability to obtain within 6 months of employment
Must demonstrate effective communication and customer service skills, human relation skills and time management skills.
PREFERRED QUALIFICATION(S):
Prefer previous experience in health care service setting, and interaction with patients and families regarding care planning.
