Geriatric Primary Care Clinic social worker serves in a consultative capacity for the Geriatric Primary Care Clinic, providing primarily consultative palliative care or palliative care case management services to Veterans referred for social work services. Through the consultation request process, the incumbent provides palliative care consultation or other necessary professional social work services to address the psychosocial needs of Veterans consulted for specific psychosocial issues. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
