This position functions as the Medical Center's Liaison for VA's HCHV Contract sites and models. The incumbent is responsible for monitoring and coordination of services provided by community-based programs funded under VA's LDSH and CERS Program along with other contracts and models to ensure that Veterans in community-based funded programs are receiving quality services. HCHV Contract sites and models, are an essential and critical part of VHA.
