The Senior Social Worker (Dialysis) performs clinical case management in End Stage Renal Disease (ESRD) care to Veterans enrolled at the Ralph H. Johnson VA Medical Center. The incumbent will be assigned to work with patient populations with high complex health or mental health problems and performs a variety of activities including outreach, multidisciplinary activities, psychosocial assessments, consultations, and educational services to medical center staff and community service providers.
