This is the full performance level for a Senior Social Worker in the Community Living Center Hospice Unit. The Community Living Center (CLC) Hospice Social Worker provides a variety of psychosocial assessments, case management, psychosocial counseling, supportive counseling, education services, differential diagnosing and treatment with Veterans who are assigned to the Inpatient Hospice Program in the Community Living Center.
