The primary responsibilities of this position are to provide social work services to Patient Aligned Care Team/General Medicine & Specialty Clinics within VA Ann Arbor Ambulatory Care Services. Primarily responsible for providing Outpatient Social Work services to Veterans receiving medical care predominately in the Patient Aligned Care Team (PACT) clinics at the Toledo CBOC, Specialty clinics, Emergency Department and as a part of SW triage walk-in clinic located within the hospital setting.
